• If you are a patient who had COVID-19 or similar viral infection and are still suffering from breathlessness more than 8 weeks after your illness, you will need to be medically assessed and referred by one of our participating services.   • If you are a patient with COPD or Asthma, you will need to be medically assessed and referred by one of the participating Airway Diseases respiratory services. Please get in touch to know who they are: [email protected]

  • If you are a Healthcare professional and would like to be able to refer your patients onto the programme, please get in touch with [email protected]. 
  • If you are a social prescribing link worker, your patient will need to have been medically assessed by their respiratory service to be referred.
